Abstract Introduction Neuropsychiatric symptoms are an integral component of the natural history of dementia, occurring from prodromal to advanced stages of the disease process and causing increased burden and morbidity. Clinical presentations are pleomorphic and clinical management often requires combinations of pharmacological and non-pharmacological interventions. However, limited efficacy and a non-negligible incidence of adverse psychotropic drug events emphasize the need for novel therapeutic options. Objectives To review the evidence supporting use of medical cannabinoids for treatment of neuropsychiatric symptoms (NPS) of dementia. Methods We conducted a systematic review of the medical literature to examine scientific publications reporting use of medical cannabinoids for treatment of NPS. Medical Subject Headings (MeSH) were used to search for relevant publications and only papers reporting original clinical information were included. A secondary search was performed within selected publications to capture relevant citations that were not retrieved by the systematic review. The papers selected were categorized according to the level of evidence generated by the studies in relation to this clinical application, i.e. (1) controlled clinical trials; (2) open-label or observational studies; and (3) case reports. Results Fifteen publications with original clinical data were retrieved: five controlled clinical trials, three open-label/observational studies, and seven case reports. Most studies indicated that use of medical cannabinoids engendered favorable outcomes for treatment of NPS related to moderate and advanced stages of dementia, particularly agitation, aggressive behavior, sleep disorder, and sexual disinhibition. Conclusion Medical cannabinoids constitute a promising pharmacological approach to treatment of NPS with preliminary evidence of benefit in at least moderate to severe dementia. Controlled trials with longitudinal designs and larger samples are required to examine the long-term efficacy of these drugs in different types and stages of dementia, in addition to their adverse events and risk of interactions with other drugs. Many pharmacological details are yet to be determined, such as dosing, treatment duration, and concentrations of active compounds (e.g., cannabidiol [CBD]/ Δ9-tetrahydrocannabinol [THC] ratio) in commercial preparations of medical cannabinoids.

ABSTRACT: Dysgeusia is understood as changes in the perception of taste in patients infected with COVID-19 and has been frequently observed in the early stages of the disease. Along with the causes of dysgeusia, we can mention the correlation with infectious conditions, being of importance for the differential diagnosis. Dysgeusia can be considered one of the first manifestations of COVID-19, being an important contribution in the search for solutions regarding early diagnosis and treatment, in addition to its hospital and laboratory application.

RESUMEN: La disgeusia se entiende como cambios en la percepción del gusto en pacientes infectados por COVID-19 y se ha observado con frecuencia en las primeras etapas de la enfermedad. Junto a las causas de la disgeusia, podemos mencionar la correlación con patologías infecciosas, siendo de importancia para el diagnóstico diferencial. La disgeusia se puede considerar como una de las primeras manifestaciones del COVID-19, siendo un aporte importante en la búsqueda de soluciones en cuanto al diagnóstico y tratamiento precoces, además de su aplicación hospitalaria y de laboratorio.

Stainless steel and nickel alloy have high corrosion resistance in high-temperature environments due to the high Cr content present in their chemical composition, being widely used in components of nuclear reactors, petrochemical industries, etc. Through proper processes and procedures, it becomes possible to join these alloys. However, this union can generate detrimental factors in its performance, among them, the residual stresses. In this work, the residual stresses generated by the autogenous GTAW process, due to different interpass temperatures on the weld bead geometry, were analyzed by the Hole-Drilling technique in dissimilar welding joints of stainless steel AISI 316L and Inconel 718 alloy. In addition, the Vickers microhardness measurements were carried out to evaluate the hardness profile in the cross section of the weld bead covering base metal (BM), heat affect zone (HAZ) and weld metal (WM). We found that in the interface region between BM and HAZ of each dissimilar joint metal, residual stresses increased above 300 MPa, while hardness increased above 160 HV.

Résumé Cette recherche se situe dans le domaine de l'enseignement de l'histoire. Nos sources sont constituées par les annales des sept Rencontres d'histoire de l'Etat de Bahia, dont les travaux publiés et les sommaires approuvés ont été intégralement consultés. Cet article cherche à analyser les communications présentées à la Anpuh-Bahia entre 2002 et 2014. L'enquête a visé de manière large les thématiques ayant rapport avec l'objet d'étude, y compris les recherches dans les domaines académique et scolaire à propos de l'enseignement de l'histoire à Bahia. Les résultats montrent un pauvre intérêt par les sujets liés à l'enseignement de l'histoire. Les textes en question ne se penchent pas sur les débats concernant l'enseignement de l'histoire, recyclant plutôt les vieilles dichotomies enseignement de l'histoire/recherche et historiographie, ou études universitaires/études secondaires.

Summary This research project is carried out in the area of the teaching of history. The primary source of this research was the annals of the seven editions of the State Meeting on History in Bahia; the works consulted being the published articles and the approved summaries. This article seeks to analyze the communications presented in the Anpuh-Bahia between 2002 and 2014. This research contemplated themes related to the object of study, including the products of research in the academic and school environment, regarding the teaching of history in Bahia. The results show that there is little information on topics related to the teaching of history. The texts do not reflect the debate about the teaching of history and show the dichotomy between the Teaching of History vs Research and Historiography, or Undergraduate degree vs Secondary school.

Resumo Esta investigação situa-se no espaço do Ensino de História. Nossa fonte primária de pesquisa foram os Anais das sete edições do Encontro Estadual de História na Bahia, consultando-se os trabalhos publicados integralmente e os resumos aprovados. Este artigo visa analisar as comunicações apresentadas na Anpuh-Bahia, entre os anos de 2002 e 2014. A pesquisa contemplou de forma ampla as temáticas relacionadas ao objeto, incluindo também as produções de pesquisa no âmbito acadêmico e escolar sobre o Ensino de História na Bahia. Os resultados mostram uma baixa expressividade de temas ligados ao Ensino de História. Os textos não expressam o debate acerca do ensino de história e representam o espelho da velha dicotomia Ensino de História versus Pesquisa e Historiografia, ou Licenciatura versus Bacharelado.

RESUMO INTRODUÇÃO: A trombose venosa é uma doença de caráter multifatorial e de alta incidência na população. O desenvolvimento dessa patologia está intimamente ligado à presença de fatores ambientais e genéticos, podendo ocorrer em associação ou isoladamente. Entre as diversas mutações genéticas que podem predispor a um evento trombótico, a de maior ocorrência na população é a mutação G1691A no fator V da coagulação, conhecida como fator V de Leiden (FVL). Essa mutação provoca o fenótipo conhecido como resistência à proteína C ativada, levando a um quadro de hipercoagulabilidade, que aumenta o risco de trombose. OBJETIVO: Investigar a presença do FVL em indivíduos com histórico de trombose venosa e, como grupo-controle, indivíduos sem histórico. MÉTODO: A metodologia utilizada foi a técnica de reação em cadeia da polimerase (PCR), seguida de polimorfismo no comprimento dos fragmentos de restrição (RFLP). RESULTADOS: Os resultados mostraram prevalência da mutação de 21,5% em heterozigose entre o grupo de pacientes; não foram identificados indivíduos homozigotos mutados. Os resultados também apresentaram alto índice de recorrência entre os portadores da mutação. CONCLUSÃO: Conclui-se que a pesquisa da mutação do fator V tem forte impacto na investigação do tromboembolismo venoso a fim de elucidar a etiologia do evento, além de auxiliar no tratamento e na profilaxia diante da recorrência.

ABSTRACT INTRODUCTION: Venous thrombosis is a multifactor disease with high incidence in the population. The development of this disease is closely linked to the presence of environmental and genetic factors and may occur in combination or alone. Among the various genetic mutations that may predispose to a thrombotic event, the most frequent in the population is the G1691A mutation in clotting factor V, known as factor V Leiden (FVL). This mutation brings the phenotype known as activated protein C resistance, leading to a hypercoagulable state, which increases the risk of thrombosis. OBJECTIVE: To investigate the presence of FVL in individuals with history of venous thrombosis, and as the control group, individuals with no history of the disease. METHOD: The method used was the polymerase chain reaction (PCR) followed by restriction fragment length polymorphism (RFLP). RESULTS: The results showed mutation prevalence of 21.5% in heterozygosity in patient group; no individuals with mutated homozygous were identified. The results also showed a high recurrence rate among the mutation carriers. CONCLUSION: In conclusion, the research of mutation on factor V has strong impact on investigation of venous thromboembolism, in order to elucidate the etiology of the event, and in treatment and in prophylaxis against the recurrence.

CONTEXTO: A Seção de Psiquiatria Geriátrica da Associação Mundial de Psiquiatria (AMP), desde 1997, vem desenvolvendo Declarações de Consenso relevantespara a prática da Psiquiatria Geriátrica. Desde 2006, a Seção vem trabalhando para desenvolver uma Declaração de Consenso sobre Ética e Capacidade em pessoas idosas com transtornos mentais. MÉTODO: Uma Conferência de Consenso foi realizada em Praga em setembro de 2008. Organizada pela Seção de Psiquiatria da Pessoa Idosa da AMP, ela contou com a participação do International Council of Nurses, Alzheimer Europe e Alzheimer Disease International. Os participantes foram reconhecidos pela sua perícia nesse domínio e vieram de 11 países. Incluíam psiquiatras, uma neurologista, um enfermeiro e representantes de cuidadores familiares. RESULTADOS: Após dois dias de reuniões e debate, redigiu-se um rascunho da declaração que foi submetida para análise nas diversas organizações/associações que participaram da reunião. Após as sugestões finais recolhidas, um texto definitivo foi preparado em inglês e publicado. A presente versão em português é da responsabilidade de dois participantes lusófonos da reunião, que são também coautores da declaração de consenso final. CONCLUSÕES: Essa Declaração de Consenso oferece aos clínicos em saúde mental que cuidam de pessoas idosas com transtornos mentais, cuidadores, outros profissionais da saúde e o público em geral as definições e o debate sobre os princípios éticos que podem frequentemente ser complexos e desafiadores, apoiados em orientações práticas para satisfazer tais necessidades e padrões éticos e encorajar a boa prática clínica.

BACKGROUND: The World Psychiatric Association (WPA) Section of Old Age Psychiatry, since 1997, has developed Consensus Statements relevant to the practice of Old Age Psychiatry. Since 2006 the Section has worked to develop a Consensus Statement on Ethics and Capacity in older people with mental disorders. METHOD: A Consensus Conference was realized in Prague, September 2008. The meeting was organized by the WPA Section of Old Age Psychiatry, with the participation of International Council of Nurses, Alzheimer Europe and Alzheimer Disease International. Participants who are recognised for their expertise in this area came from 11 countries and include psychiatrists, a neurologist, a nurse and family caregiving representatives. RESULTS: After two days of debate a draft was prepared and submitted to the organizations/associations represented at the meeting. When their respective comments were collected, a final text was prepared and published originally in English. This Portuguese version was prepared by two members of the meeting who came from Portuguese spoken countries. DISCUSSION: This Consensus Statement offers to mental health clinicians caring for older people with mental disorders, caregivers, other health professionals and the general public the setting out of and discourse in ethical principles which can often be complex and challenging, supported by practical guidance in meeting such ethical needs and standards, and to encouraged good clinical practice.

INTRODUÇÃO: O efeito do envelhecimento nas concentrações plasmáticas em estado de equilíbrio da paroxetina foi investigado em 136 pacientes com depressão, tratados com doses variando entre 10 e 120 mg/dia de paroxetina. MÉTODOS: Os pacientes foram separados em três grupos: com idade até 64 anos (idade média ± desvio padrão: 41,7±12,6 anos; n = 44), entre 65 e 79 anos (72,8±4,7 anos; n = 64), e com mais de 80 anos (82,8±3,3 anos; n = 28). As doses de paroxetina foram normalizadas para 20 mg/dia. Amostras de sangue foram coletadas em condições de estado de equilíbrio. Os níveis plasmáticos de paroxetina foram medidos por cromatografia gasosa - espectrometria de massa. As funções hepática e renal foram medidas por testes laboratoriais clínicos estandardizados. RESULTADOS: Uma grande variabilidade interindividual dos níveis plasmáticos de paroxetina (37 vezes) foi medida para uma mesma dose. A média dos níveis plasmáticos de paroxetina corrigida para uma dose diária de 20 mg foi 87% maior no grupo com mais de 80 anos (56,4±64,1 ng/mL; p < 0,05) e 57% maior no grupo com idades entre 65 e 79 anos (46,7±33,4 ng/mL; p < 0,001) quando comparada com a média dos pacientes com menos de 64 anos (29,9±11,9 ng/mL). A idade foi correlacionada significativamente com os níveis plasmáticos de paroxetina (r = 0,21, p < 0,05) CONCLUSÃO: A redução de doses da paroxetina em pacientes idosos parece justificada.

INTRODUCTION: The effect of aging on steady-state plasma concentrations of paroxetine was investigated in 136 depressive patients treated with paroxetine 10-120 mg/day. METHODS: The patients were divided into three groups: aged up to 64 years (mean age ± standard deviation: 41.7±12.6 years; n = 44), between 65 and 79 years (72.8±4.7 years; n = 64), and 80 years or older (82.8±3.3 years; n = 28). Paroxetine doses were normalized to 20 mg/day. Blood samples were collected under steady-state conditions. Paroxetine plasma levels were measured by gas chromatography/mass spectrometry. Hepatic and renal functions were measured by standardized clinical laboratory tests. RESULTS: A large interindividual variability of paroxetine plasma levels (37-fold) was measured for a given dose. The mean plasma levels of paroxetine corrected for a 20 mg daily dose were 87% higher in the very elderly (≥ 80 years) patients (56.4±64.1 ng/mL; p < 0.05) and 57% higher in the elderly (65-79 years) patients (46.7±33.4 ng/mL; p < 0.001) when compared to the adult patients (< 64 years) (29.9±11.9ng/mL). Age correlated significantly with paroxetine plasma levels (r = 0.21, p < 0.05). CONCLUSION: Recommended dose reduction of paroxetine in elderly patients seems therefore justified.

A Organização Mundial da Saúde (OMS) e a Seção de Psiquiatria da Pessoa Idosa da Associação Mundial de Psiquiatria (AMP), em colaboração com um grupo interdisciplinar de representantes das principais associações internacionais e organizações não-governamentais implicadas na saúde mental das pessoas idosas, publicaram três declarações técnicas de consenso sobre a psiquiatria da pessoa idosa (1), a organização dos cuidados em psiquiatria da pessoa idosa (2) e o ensino da psiquiatria da pessoa idosa (3). O Dia Mundial da Saúde 2001, cujo tema foi "Não à exclusão, sim aos cuidados", deu origem a uma nova reunião de consenso sobre o tema da estigmatização e discriminação das pessoas idosas com transtornos mentais. Essa nova reunião foi realizada em Lausanne nos dias 8 e 9 de outubro de 2001 e produziu uma declaração técnica de consenso. O texto inicial foi publicado pela OMS e AMP em inglês. Este artigo apresenta a versão em português desse documento.

This technical consensus statement is jointly produced by the Old Age Psychiatry section of the World Psychiatric Association and the World Health Organization, with the collaboration of several NGOs and the participation of experts from different regions. It is intended to be a tool for (i) promoting debate at all levels on the stigmatization of older people with mental disorders; (ii) outlining the nature, causes and consequences of this stigmatization; and (iii) promoting and suggesting policies, programs and actions to combat this stigmatization.

Diffuse intestinal pneumatosis appears as gaseous cysts in the intestinal wall in the submucosal layer or, more frequently, sub-serous layer. It affects more the jejunum than the ileum, can be diffuse and may extend into the colon. It's diagnosis is established during a small bowel transit examination, necropsy examination or during surgical exploration of the peritoneal cavity.
